JOB DESCRIPTION

Based within an integrated Community Health and Care Partnership covering West Glasgow, you will be responsible for managing the planning and performance management function for the CHCP including Community Care, Addictions and Children’s services, people with chronic disease and other primary care services. Working closely with staff, service users, carers and partner agencies, you will play a key role in driving forward improvements in health and social work services.
